Farm Bureau Financial Services offers three convenient ways to report an auto, home or property insurance claim. You can either 1.) report a claim online, 2.) call our 24-hour claims center, or 3.) contact your agent.
For car insurance claims, homeowners insurance claims, property insurance claims or commercial insurance claims in Arizona, Iowa, Kansas, Minnesota, Nebraska, New Mexico, South Dakota and Utah, contact your agent or call our 24-hour claims center toll-free at 1-800-226-6383.
Outside of Arizona, Iowa, Kansas, Minnesota, Nebraska, New Mexico, South Dakota and Utah, please contact your agent.
For faster resolution, please be prepared to provide the following information:
- Insured's Name
- Address of Insured
- Policy Number
- Date of Damage or Injury

Life Insurance or Annuity Claims
If the policyholder had a policy through Farm Bureau Life Insurance Co. in West Des Moines, Iowa and lived in Arizona, Colorado, Idaho, Iowa, Kansas, Minnesota, Montana, Nebraska, New Mexico, North Dakota, Oklahoma, South Dakota, Utah, Wisconsin or Wyoming, contact their agent or call the claims center toll-free at 1-800-247-4170 option 1, 1, 2 between 8 a.m. and 4:30 p.m. Central time.
If the policyholder did not live in one of the states listed above, please call the claims center toll-free at 1-800-247-4170 option 1, 1, 2 between 8 a.m. and 4:30 p.m. Central time.
For faster resolution, please have the following information available when you call the claims center or meet with an agent:
- Policy or contract number
- Certified death certificate
- Beneficiary Statement - located on the right-hand side of this page. Please note the state variations.
- Payment Option Election form - located on the right-hand side of this page. This form is only required if a Payment Contract Option is selected in item C1 in the Beneficiary Statement. If you prefer, you may complete this form online prior to printing it.

Identity Theft Report or Inquiry
Whether you have identity theft coverage through your personal property/liability policy (Farm Bureau Member's Choice) or through your commercial agriculture policy (AgMax), please call 1-866-399-3237 Monday through Friday, 7:30 a.m. - 6 p.m. or Saturday 8:00 a.m. till noon Central time or contact your agent to report an identity theft incident or make an inquiry. You may also call to inquire about a proactive fraud alert or with questions about services provided through this coverage. Please have your policy number available when you call.
